-Duly accomplished Application Form with SAG
-Two (2) passport-size photos (white background, with collar, studio-taken)
-Certificate of Employment or Endorsement Letter from the industry/institution (for employed industry workers/professionals)
-Photocopy of Birth Certificate or Marriage Certificate (for married women)
-One (1) long brown envelope

Applicants must not already hold a Bookkeeping NC III National Certificate.
