👀 Eyes Here CHS USMCEE Confirmed Applicants:

1. Please be advised that before you will be officially enrolled, a face to face interview will be conducted (specific date will be posted separately) to be held at the College of Health Sciences.

2. The following are required Laboratory test to be presented during interview:

Urinalysis
Fecalysis
Chest X-Ray with Reading
Hepatitis B Test
Pregnancy Test (for female)
Neuropsychiatric Exam Result

3. Please take note that the Neuropsychiatric Test Results must come from an accredited Hospital and or an Out Patient Department Clinic who has a Licensed Psychiatrist.

4. Laboratory Tests and Neuropsychiatric Exam Results should not be more than 1 month from the test date.

5. A Medical Certificate certifying the present medical condition of the applicant.

Note: The Department and College has the right not to accept results deemed coming from questionable testing areas.

6. All requirements must be original and must be placed in a long clear envelop together with 2 pcs passport size with name and 2 pcs 2x2 picture with white background.

7. A Parent and or a Legal Guardian is requested to accompany the applicant for additional important information of department policies.
