TO THOSE APPLYING FOR THE SOCIAL AMELIORATION PROGRAM BY OUR NATIONAL GOVERNMENT, HERE ARE THE FOLLOWING ELIGIBLE BENEFICIARIES AND THE NEEDED REQUIREMENTS (According to Joint Memo Circular No. 1 series of 2020):

👉Senior Citizens
• Senior Citizen’s ID

👉Persons With Disability (PWD)
• PWD ID
• Certificate of Separation from or Suspension of Work

👉Pregnant Women / Lactating Mothers
• Valid ID
• Certification from Rural Health Unit; ukon Birth Certificate of Child

👉Solo Parents
• Solo Parent’s ID
• Certificate of Employment / Certificate of Separation from or Suspension of Work

👉OFWs in distress
• Valid Passport Bio Page
• Copy of any of the following:
1. Passport arrival stamp
2. Proof of departure ticket
3. Overseas Employment Certificate
4. Employment Contract

👉Homeless Families and Indigenous Peoples (IPs)
• Certification from Barangay or from MSWDO nga gapatunay nga homeless or IP
• Certification from NCIP or the Tribal Chieftain

👉Workers in the Informal Sector / Self Employed

• Driver’s License
• TODA ID
• Certification of Membership from the Organization
• Certificate of Public Convenience (CPC)

▶️For Kasambahay, laundry maid, pakyaw workers, construction workers:
• Employment ID or Kasambahay ID
• Certificate of Employment
• Certificate of Separation from or Suspension of Work

▶️For Small business owners:
• Proof of Business
• Barangay Clearance
• Any other document showing employment / enterprise

👉For Employers:
• Establishment Report
• Company Payroll

👉For Rice Farmers
• RSBSA Enrollment Stub (for Cash Assistance)
• Notice of Cash Grant (for SURE Aid)
• Any government-issued ID with photo
• Duly accomplished simplified Promissory Note (PN)

👉For Service-Conduit Borrower
• Loan Application
• List of Eligible Rice Farmers
• Board Resolution for Financial Assistance
• Identified GFI
• Standard PN
• Authority to Debit Deposit Account

All barangays are hereby mandated to facilitate the issuance of the barangay clearance, FREE OF CHARGE, for the applicable beneficiaries stated above.

Note: As of the moment, WALA PANG FORMS. Local DSWD will be the one to reproduce the forms for distribution. Wala pang available forms sa barangay.
