📌𝗡𝗢𝗧𝗘: The applicant for this grant must be a Filipino citizen and must NOT be existing grantees of any CHED-sponsored or any funded scholarship programs.

📍Secure the following documents to be submitted at the Office of the Student Affairs and Services (OSAS):

2. Two (2) copies of Certificate of Indigency with a combined household (parents/guardians) gross annual income not exceeding Four Hundred Thousand Pesos (PhP 400,000.00), duly issued by the Barangay (one [1] original copy and one [1] photocopy).

3. Two (2) copies of the Certificate of Registration (COR) for the First Semester, AY 2026-2027, duly signed by the Registrar and Accountant (one [1] original copy and one [1] photocopy).

4. Two (2) copies of the photocopy of the School ID (front and back on one page, back-to-back) with 3 signature specimen.

5. Two (2) copies of the latest 2×2 ID picture with a white background.
