If your name is on the list, proceed to Step 2.

𝗦𝗧𝗘𝗣 𝟮. Proceed to Table 1 to get a claim stub (QUEUE NUMBER)

𝗦𝗧𝗘𝗣 𝟯. Proceed to Table 2 for the release of check and signing of vouchers. Please bring photocopies of the following:
a. Student’s School ID
b. Valid ID of Parent/Guardian

𝗦𝗧𝗘𝗣 𝟰. Sign the General Payroll Form TES 1 on Table 3 and submit the required number of colored photocopies of the student’s school ID, front and back, with 3 specimen signatures in blue ink.
