•Original Report Card (SF9)
•For Transferees: Official Transcript of Records for Evaluation Purposes with NSTP Serial Number for those who completed the NSPT 1 and 2 courses and Certificate of Transfer Eligibility
•Original Certificate of Good Moral Character
•Photocopy of PSA Birth Certificate
•6pcs-1x1 Colored ID Picture (White Background)
•1pc-2x2 Colored ID Picture (White Background)

2. Medical Exam are conducted by the school's accredited medical facilities. Referral and schedule are provided by the School Clinic.
