• Updated Bio-Data
• Certified Copy of the Candidate's TOR or Copy of Grades from the Previous Semester immediately prior to the Filing of Candidacy (Authenticated by the Candidate's Adviser)
• Sworn Certification for Unreleased Grades – in cases where grades have not yet been released, a sworn certification from the subject teacher is required stating that the candidate has no failing or incomplete (INC) remarks
• Certificate of Enrolment or Registration Form
• Certificate of Good Moral Character
• Accomplished Affidavit of Undertaking (For Graduating Students)*
• Additional requirements included
• ID photo (to be used in the Voting Panel)
• Photo proof with Certificate of Candidacy
