Additional Reminders for the Departmental Orientation
Please be informed that attendance will be checked twice during the orientation.

* Attendance (sign-in) will be checked on or before the start of the Departmental Orientation.
* Attendance (sign-out) will be checked after your respective year-level orientation.

Please make sure to remain until the end of your assigned session. For example, 1st year students may sign out only after their year-level orientation at 2:30 PM. Students who leave before the official dismissal will be considered to have incomplete attendance.
In addition, all students are required to sign an undertaking acknowledging that they have attended the orientation and understood the department’s policies, rules, and regulations.

This document will be notarized and will serve as an official record of your commitment to comply with departmental policies.
Failure to attend the orientation, complete the attendance requirements, or sign the undertaking will result in the corresponding consequences in accordance with departmental policies.
