Here's your guide on how to obtain Medical Certifical for On-the-Job Training (OJT) 🫰🏻

Step 1: Submit an endorsement letter from your College/Department stating the reason, date(s), location etc.
* Pls attach list of students
* Pls address your letter to Dr. Muhammad M. Puting

Step 2: Ask your specific company their requirements and their standard list of laboratory results needed to be attached to your medical certificate.

Step 3: Once done with no. 2, pls proceed to clinic to ask for Lab requests to be submitted to your choice of laboratory facility.

Step 4: Photocopy all lab results and submit it to clinic for further evaluation and assessment.

Step 5: Releasing of Medical Certificate

Done done done. Congrats and Goodluck on your journey 😁😂

"Hyperuricemia is an excess or abnormally high level of uric acid in the blood (>6.8 mg/dL). Hyperuricemia is associated with gout, CVD, and new onset diabetes.
