2. Original SF 9/Form 138 (Report card) in Senior High School;⁣
3. Original Certificate of Good Moral Character in Senior High School;⁣
4. Clear photocopy of PSA/NSO Birth certificate. In case when PSA/NSO Birth certificate is not clear, please submit clear photocopy of Local Birth Certificate provided by the Local Civil Registrar;⁣
5. Clear photocopy of PSA/NSO Marriage Certificate (for female married students). In case PSA/NSO Marriage Certificate is not clear, the student must submit Local Marriage Certificate;⁣
6. Original Barangay Certificate of Residency;⁣
7. 1 long plastic transparent envelope.⁣
8. 1 pc. 2x2 picture with nametag (white polo shirt attire) (attached to Student’s Medical Information Form) ⁣
9. 1 pc. long brown kiwi envelope indicating Surname, First name, Middle Initial & Program.⁣
10. Photocopy of Person With Disability (PWD) Identification Card (for PWD students)⁣
11. Original X-ray result with abstract.⁣
12. Original copy of drug test result.⁣

Please be advised that applicants who were unable to take the College Admission Test last April 28 may take the exam this Friday, May 29, 2026, during the morning session of the said examination.

Furthermore, walk-in applicants who wish to enroll in our institution may submit their documentary requirements (please see the previous post for the list of requirements) and may also take the College Admission Test.

Per MDRRMO Memorandum No. 05, Series of 2026, the suspension of classes has already been lifted.

However, due to the continued use of classrooms and other school facilities by uniformed personnel from the Philippine National Police (PNP) and Bureau of Fire Protection (BFP) as temporary quarters while assisting in ashfall clearing operations, face-to-face classes will remain suspended.

All faculty and students are hereby advised that classes will shift to online modality starting tomorrow, May 5, 2026, until further notice.
