Good morning Students. Please be advised to follow proper order when submitting your documents.
1. Please put your documents inside a brown envelope.
2.Label it with your SURNAME, FIRST NAME then followed by your MIDDLE NAME/MIDDLE INITIAL for example: *DELA ,CRUZ, JUANA Z.
3.Write your COURSE, YEAR LEVEL AND MAJOR . BSEd-Mathematics 1
4.Identify your documents submitted at the back of your envelope.
5. Each student should submit 5.1.prospectus with Grades in the Old Normal ( Face-2-Face) 5.2. Enrollment form
