For Senior High School Graduate
1. Applicant Photo
2. Certificate of Good Moral Character
3. Senior High School Card
4. PSA copy of Birth Certificate

For ALS Passer
1. Applicant Photo
2. Certificate of Good Moral Character
3. Certified True Copy of List of Passer
4. Certificate of Rating with remarks equivalent to senior highschool level/graduate and date of graduation
5. PSA copy of Birth Certificate

For Transferee
AVAILABILITY OF SLOTS
1. Applicant Photo
2. Certificate of Good Moral Character
3. Copy of Grades / Transcript of Record (Must have NO dropped and failing grade)
4. Honorable Dismissal
5. PSA copy of Birth Certificate

GRADUATE PROGRAM (For Masters and Doctoral)

For Professional Education

Please take note of the following requirements per program:
1. GWA Required for College of Education applicants is 85%.
2. For BS Hospitality Management applicants, Height requirement for Men is 5'6" and above and for Women is 5'4" and above

Posted here are the enrollment schedule, enrollment procedure for new and old students and offered subjects, for 1st Semester S.Y. 2020 - 2021.
