STEP 1- ADMISSION OFFICE
Requirements:
1. Photocopy of Form 138/High School Card with complete grades (for freshmen) or a Photocopy of the Transcript of Records (for Transferees).
2. Photocopy of the Certificate of Good Moral Character indicating that the applicant is a graduate (for freshmen) or Photocopy of the Certificate of Good Moral Character from the last school attended (for transferees).
3. Original Ishihara test result.
4. Original Police clearance.
5. Two (2) pieces of 2”x2” pictures.
Procedures:
o Fill out and submit the Personal Data Sheet (PDS) form.
o Undergo an interview.
o Secure an Application for Admission form.
o Secure an enrollment slip.

STEP 2- GAD OFFICE
Requirements:
1. Original and two (2) photocopies of the CBC test result.
2. Original and two (2) photocopies of the Chest x-ray result.
3. Original and two (2) photocopies of the drug test result.
4. Original and two (2) photocopies of urinalysis test result.
5. Original and two (2) photocopies of blood typing result.
6. One (1) 6”x9” envelope.
Procedures:
o Fill out the Medical History form.
o Have your vitals signs checked by the Academy Physician/ Campus Nurse.
o Have your test results signed by the Academy Physician/ Campus Nurse.

STEP 3- REGISTRAR’S OFFICE
Requirements:
1. Original Form 138/High School Card with complete grades (for freshmen) or the Original Transcript of Records (for Transferees).
2. Original Certificate of Good Moral Character indicating that the applicant is a graduate (for freshmen) or Original Certificate of Good Moral Character from the last school attended (for transferees).
3. Original PSA Birth Certificate.
4. Filled-out Application for Admission Form (secured from the Admission Office).
5. Photocopy of the CBC result (signed by the Academy Physician/ Campus Nurse).
6. Photocopy of the Chest x-ray result (signed by the Academy Physician/ Campus Nurse).
7. Photocopy of the Drug test result (signed by the Academy Physician/ Campus Nurse).
8. Photocopy of the Urinalysis test result (signed by the Academy Physician/ Campus Nurse).
9. Photocopy of the Blood typing result (signed by the Academy Physician/ Campus Nurse).
10. Two (2) pieces of 2”x2” pictures.
11. Two (2) pieces long brown envelopes.
12. Honorable Dismissal (for Transferees only).
Procedures:
o Undergo a short interview for data gathering.
o Enroll in your preferred class schedule.
o Secure a Certificate of Registration (COR).
o Secure a Letter of Request for Form 137.

STEP 4- FACULTY ROOM
Requirements:
1. Certificate of Registration (COR).
2. Filled-out ID QR Code or filled-out ID slip.
Procedure:
o Secure your School ID.

STEP 5- LIBRARY
Requirements:

The College Placement Exam (CPE) will be held on June 1, 2, and 3, 2026, from 8:00 AM to 12:00 NN and 1:00 PM to 4:00 PM.

Filing of applications will be on May 18, 19, 20, 26, 27, and 28, 2026, at the Admission Office, NAAP Palmayo Campus.

Requirements:

The Office of Student Affairs strictly enforces the “𝗡𝗼 𝗜.𝗗., 𝗡𝗼 𝗘𝗻𝘁𝗿𝘆” policy.

𝗔𝗹𝗹 𝗲𝗻𝗿𝗼𝗹𝗹𝗲𝗱 𝘀𝘁𝘂𝗱𝗲𝗻𝘁𝘀 are 𝗿𝗲𝗾𝘂𝗶𝗿𝗲𝗱 𝘁𝗼 𝘀𝗲𝗰𝘂𝗿𝗲 𝘁𝗵𝗲𝗶𝗿 𝘂𝗽𝗱𝗮𝘁𝗲𝗱 𝗶𝗱𝗲𝗻𝘁𝗶𝗳𝗶𝗰𝗮𝘁𝗶𝗼𝗻 𝗰𝗮𝗿𝗱𝘀 before the start of every academic year or semester in order to enter the school premises.

𝗦𝘁𝘂𝗱𝗲𝗻𝘁𝘀 𝗺𝘂𝘀𝘁 𝗿𝗲𝗺𝗲𝗺𝗯𝗲𝗿 𝘁𝗵𝗲 𝗳𝗼𝗹𝗹𝗼𝘄𝗶𝗻𝗴:
- 𝗔𝗹𝘄𝗮𝘆𝘀 𝘄𝗲𝗮𝗿 𝘁𝗵𝗲𝗶𝗿 𝗶𝗱𝗲𝗻𝘁𝗶𝗳𝗶𝗰𝗮𝘁𝗶𝗼𝗻 𝗰𝗮𝗿𝗱𝘀; forgetting to wear the I.D. will 𝗡𝗢𝗧 be accepted as a valid excuse.
- Security personnel are 𝗔𝗨𝗧𝗛𝗢𝗥𝗜𝗭𝗘𝗗 𝗧𝗢 𝗗𝗘𝗡𝗬 𝗘𝗡𝗧𝗥𝗬 to any student who fails to present their I.D., in accordance with the agreement between the Dean of the Office of Student Affairs and the Head of the Campus Safety and Security Office.

To streamline the application process, the Office of the Registrar has implemented a more convenient method for requesting school I.D.s. Students may scan the QR code posted outside the Registrar’s Office and fill out the required information via the provided Google Form. Once submitted, the form will automatically reflect in the system, and students may proceed to the Registrar’s Office to claim their I.D.

𝗜𝗻 𝗰𝗮𝘀𝗲 𝗼𝗳 𝗮 𝗹𝗼𝘀𝘁 𝗜.𝗗.:
- The student must present an 𝗮𝗳𝗳𝗶𝗱𝗮𝘃𝗶𝘁 𝗼𝗳 𝗹𝗼𝘀𝘀 to be allowed entry into the school premises. The affidavit 𝗺𝘂𝘀𝘁 𝗮𝗹𝘀𝗼 𝗯𝗲 𝘀𝘂𝗯𝗺𝗶𝘁𝘁𝗲𝗱 𝘁𝗼 𝘁𝗵𝗲 𝗢𝗳𝗳𝗶𝗰𝗲 𝗼𝗳 𝘁𝗵𝗲 𝗥𝗲𝗴𝗶𝘀𝘁𝗿𝗮𝗿 in order to secure a replacement copy of the school I.D.
