SCHEDULE OF ADDING AND DROPPING OF SUBJECTS

Students who wish to add or drop subjects are advised to visit the Registrar’s Office first to secure their Adding/Dropping Form before proceeding to their respective Program Adviser for proper assessment of the subjects that need to be added or dropped.

Note: The drug test and pregnancy test must be taken during the last week of October to the first week of November. These documents are not required during enrollment but must be submitted within the prescribed testing period.

3. Medical Certificate – Original and one (1) photocopy
4. 2x2 ID Photo – Two (2) copies
5. PSA Birth Certificate – Original and one (1) photocopy
6. PSA Marriage Certificate (if married) – Original and one (1) photocopy
7. Certificate of Indigency
8. Two (2) long brown envelopes
9. TOR for evaluation (Original and 1 Photocopy)
10. Honorable Dismissal (Original and 1 Photocopy)

Transfer students with lacking documents will not be entertained.

DCPC Enrollment Procedure for Incoming First-Year Students
Enrollment Schedule: June 29 to July 3, 2026
Step 1
1. Download the following

2. Print and fill out the downloaded form.
3. Access your admission account and pre-register.
4. Download and print the Report of Rating found in your admission account.

Step 2

Passed the interview for the chosen program (For English, passed the qualifying exam)
Present the Report Card for evaluation
Present the Enrollment Slip
If you fail to pass the interview, you may consider the other programs.

Step 3

Present the Enrollment Slip

Submit the following requirements
Form 138 (Original and 1 Photocopy)
Good Moral Certificate (Original and 1 Photocopy)
Drug Test Result (Original and 1 Photocopy)
Medical Certificate (Original and 1 Photocopy)
2x2 ID (2 copies)
PSA Birth Certificate (Original and 1 Photocopy)
PSA Marriage Certificate if married (Original and 1 Photocopy)
Barangay Certificate of Residency (Original and 1 Photocopy)
Two (2) long Brown envelopes
Write your name at the Upper left of each envelope

Step 4

Present the Enrollment Slip to the following offices in any sequence for fees.

SCOTE (Under College of Teacher Ed)
CCSC (Under College of Criminal Justice)
ASSESSMENT
CLUB Membership

Step 5

Present the Enrollment Slip with a note “Qualified for ___________” to the program encoder
BEED (IT Office)
BSED (Old Registrar’s Office)
CRIMINOLOGY (IT Office)
Wait for the Printed COR
Submit the COR to the registrar with a note, “REGISTRAR’S COPY.”
Keep the COR with a note “STUDENT’S COPY” for personal Copy.

All incoming freshmen are advised to complete the form as accurately and completely as possible. Please double-check all information before submitting.

After successfully submitting your pre-registration, make sure to TAKE NOTE OF YOUR REFERENCE NUMBER. You will need this for the enrollment process next week.
