1. Nursey: 4 yrs. old and a PSA Birth Certificate
Kinder: 5 yrs. old or turning 5 yrs. old until October 31, 2023 and a PSA Birth Certificate
2. Accomplished Enrolment form
3. Medical Certificate

For Grades 1-6 Transferees:
1. SF 10 - Form 137
2. SF 9 - Report Card w/ LRN
3. PSA Birth Certificate
4. Good Moral Certificate
5. Kindergarten Certificate
6. Accomplished Enrolment form
7. Medical Certificate

For Old Students:
1. SF 9 - Report Card w/ LRN
2. Accomplished Enrolment form
3. Medical Certificate
