1. Personal Data Sheet
- Fill out the form completely.
- Attach one (1) 2x2 ID picture.
- Submit the form in good condition - not crumpled, fold or
torn.

2. Teacher Evaluation
- Access the Teacher Evaluation through the DCC Portal.
- Make sure to evaluate all your teachers.

As part of the Guidance Office’s requirements for all 1st to 4th year students of Davao Central College, you are required to fill out the Personal Data Sheet (PDS). This will allow us to update your personal information in our records and ensure that we have the necessary details whenever you avail of any Guidance Center services.

Please accomplish the PDS as soon as possible. When submitting, make sure that your completed form is clean and not torn, crumpled, or damaged in any way, as it will be required for your clearance signing.

📌 Note: In the portion asking for your high school's name and address, you may write the names and addresses of your Junior High School and Senior High School.

Alang sa mga eskwelahan nga gustong maka-avail sa maong serbisyo, pwede mo mokontak o mudiretso sa pinakaduol nga CSWDO District Office sa inyong lugar.

Ang maong serbisyo nagatuyo sa paghatag og emosyonal ug sikolohikal nga tabang alang sa mga tawo ug grupo nga naapektuhan sa linog nga nahitabo kaganiha. (PIRCS)

Office of the School President

After the earthquake this morning, the school buildings have been thoroughly inspected by our in-house engineer and officers from the Bureau of Fire and Protection and Barangay Disaster Risk Reduction and Management. We are pleased to inform the DCC Community that all buildings have been declared stable. However, due to aftershocks, the school management has decided to cancel school operations tomorrow, October 11, 2025, in both the DCC Main and Basic Education Campus. Please standby for further advisory.

We continue to pray for the safety of everyone.
