Inside your brown envelope, submit the following:

1. Guiya sa mga Ginikanan (Kopya Alang sa Skwelahan) WITH SIGNATURE of parent/ guardian and learner.

2. Answer sheets: 10 Sets. (1 Fil, 1 Eng, 1 Math, 1 Sci, 1 AP, 1 ESP, 1 TLE, 1 Music, 1 Health, 1 PE)

3. Gr. 9 Silent Reading Test (Use the provided answer sheet.)

4. Phil-IRI Filipino (Use the provided answer sheet.)

FOR TRANSFEREES:
1. Original Report Card
2. Photocopy of PSA Birth Certificate
