Requirements:
✅Fill-out Basic Education Form
✅Photocopy of PSA/NSO Birth Certificate
✅Must be 5 yrs old by June 1, 2025 (KINDER only)
✅Learners who will turn 5 yrs old by June 2 - August 31 will be accepted if they passed the ECCD Assessment (KINDER only)
✅Medical Assessment/Diagnosis (SNED only)
✅School Report Card (Form 138)/ Form 137 from previous school attended (Transferees)

ALS Requirements:
✅Fill-out Modified ALS Enrollment Form
✅Out-of-School children 12 yrs old & up for Elementary
✅Out-of-School youth 16 yrs old & up for Junior High School
