💙 Submit the GS Admissions Online Form and upload the following:
• 2x2 Latest ID Photo (white background)
• PSA/LCR Birth Certificate
• Official Report Card (SF9) at least 2nd Quarter (for incoming Grades 1–6)
• Certificate of Good Moral (for incoming Grades 1–6, if available)
💙 Proof of Admissions Fee Payment (pay at the GS Cashier)
