The Application for Leave of Absence form is now available. You may download the form in this post.

Kindly do the following:
1. Print out the Leave of Absence form on A4-sized paper.
2. Fill out only the "Student Information" and "Leave of Absence.”

Information. Please do not fill out the "TES/TDP Award Number".

3. Have your form signed by the program head, dean, and registrar.
4. Submit your fully accomplished forms at the UM SAO Matina campus.

Note: If you have already graduated, please disregard this announcement.

We are open every Monday to Friday from 8:00 AM to 5:00 PM, and on Saturday from 8:00 AM to 12:00 PM (except Holidays).

REMINDER:
• Failure to submit the form may result in termination of your grant.
• The grantee must enroll for at least one (1) semester per school year to continuously avail the said grant
