ATTENTION: UNIFAST TES 3B GRANTEES BATCH 2022 (PLEASE SEE ATTACHED LISTS)

You are requested to SUBMIT documentary requirements and SIGN the liquidation reports starting June 3, 2026 until June 27, 2026 at UM Matina Campus - Student Accounting Office (SAO) - Scholarship Section. Our office is open Monday to Friday (except holidays) from 8:00 AM to 5:00 PM only.

Requirements:

1. TWO (2) photocopies of any of the following valid IDs (front & back);
-STUDENT ID
-ALUMNI ID
-ANY GOVERNMENT ISSUED ID (PRIMARY)
Please affix your three (3) specimen signatures. Indicate your email address and active contact number. (A4-sized BONDPAPER)

2. Bring your own BLUE BALLPEN.
