Please be advised that the Office of Vicissitude will be closed for all transactions from December 18, 2025 to January 10, 2026 in observance of the Christmas break.

✓ Signed authorization letter from the subscriber.
✓ Photocopy of the subscriber’s valid ID with signature.
✓ Photocopy of the representative’s valid ID with signature.

In adherence to the directive of the Provincial Government of Cavite and the advisory of the University, all walk-in transactions at the Office of Vicissitude will be temporarily suspended from October 15 to 18, 2025.

During this period, operations will shift online. For inquiries or concerns, please reach us through our official online channels.

Please be informed that the Office of Vicissitude will be closed tomorrow, Tuesday, 𝐒𝐞𝐩𝐭𝐞𝐦𝐛𝐞𝐫 𝟐𝟑, 𝟐𝟎𝟐𝟓, in line with the campus-wide suspension of office operations due to the Enhanced Southwest Monsoon (Habagat) and Super Typhoon Nando.

Please be reminded of the following holidays for August 2025 based on Proclamation 727:
