2. Certificate of Good Moral
3. Resume
4. Certification of Grades for this Academic Year
5. Certificate of Participation in this Academic Year's General Membership Meeting and/or National Convention

To qualify as a candidate:
1. Be a JPMAP Member
2. Attended this Academic Year's General Membership Meeting or National Convention
3. Have at least an average grade of 85% or its equivalent with no failing grade this A.Y. 2018-2019
