• Photocopy ng PSA Birth Certificate
• Accomplished Early Registration Form
• Proof of previous grade (Report Card / Form 138, kung naaangkop)

𝙄𝙢𝙥𝙤𝙧𝙩𝙖𝙣𝙩 𝙍𝙚𝙢𝙞𝙣𝙙𝙚𝙧𝙨:
📌Ang mga batang papasok ng 𝗞𝗶𝗻𝗱𝗲𝗿𝗴𝗮𝗿𝘁𝗲𝗻 ay dapat na 𝟱 𝘁𝗮𝗼𝗻𝗴 𝗴𝘂𝗹𝗮𝗻𝗴 sa o bago ang Oktubre 31 ng pasukang taon.
📌Maaaring tanggapin ang mga batang 𝗺𝗮𝗴𝗹𝗶𝗹𝗶𝗺𝗮𝗻𝗴 𝘁𝗮𝗼𝗻𝗴 𝗴𝘂𝗹𝗮𝗻𝗴 𝗺𝘂𝗹𝗮 𝗡𝗼𝗯𝘆𝗲𝗺𝗯𝗿𝗲 𝟭 𝗵𝗮𝗻𝗴𝗴𝗮𝗻𝗴 𝗗𝗶𝘀𝘆𝗲𝗺𝗯𝗿𝗲 𝟯𝟭 kung natutugunan nila ang isa sa mga sumusunod:

1. Nakatapos ng isang (1) taon ng ECCD Program.

Kailangang magsumite ang magulang/tagapag-alaga ng Certificate of Completion/Attendance mula sa kinikilalang pampubliko o pribadong Child Development Center o Learning Center o;

2. Sumailalim sa Philippine Early Childhood Development (ECD) Checklist assessment.

Isasagawa ito ng Kindergarten teachers ng paaralan sa panahon ng enrollment hanggang unang linggo ng pasukan. Ang resulta ang magiging basehan ng pagtukoy sa kahandaan ng bata at ilalakip sa opisyal na record ng mag-aaral.
