“Makapag-Aral ay Karapatan Mo. Magpalista!”
Hinihikayat po ang lahat ng mga magulang at mag-aaral na papasok sa darating na S.Y. 2023-2024 sa Kindergarten at Grade 1 na magpalista mula Mayo 10 hanggang Hunyo 9, 2023.
Magpunta lamang po sa Paaralang Elementarya ng BAMBANABA upang makakuha / makapagfill up ng Enrollment Form at dalhin ang mga dokumentong kinakailangan.

MAHALAGANG PAALALA:
Ang mga papasok na Grades 2-6 ay pre-registered na at hindi na kailangang mag-early registration.
Sa Pagfill up po ng Enrollment form huwag kakalimutan na ilagay ang active cellphone number at messenger account sa dulo ng form upang mas madali po namin kayo macontact para sa mga updates.
Sa mga bata pong hindi pa nakarehistro o wala pong birth certificate. Maaari po kayong humingi ng Barangay Certificate na nagpapatunay ng mga impormasyon patungkol sa inyong anak. Kung saan nakalahad dito ang kanilang buong pangalan, kaarawan, edad, lugar ng tirahan at pangalan ng mga magulang. (temporary lang po itong tatangapin kailangan pa rin po ninyo ayusin ang kanilang birth certificate bago magsimula ang pasukan.)
Ipinapatupad pa rin po natin ang Health Safety Protocols sa loob ng paaralan para sa kaligtasan ng lahat.

Applicants must submit the following requirements:👇
1. Letter of Intent or Letter of Application addressed to the Head of Office;
2. Duly accomplished PDS (CS Form No. 212, Revised 2017) in two copies with the latest 2x2 pictures;
3. Certified photocopy of valid and updated PRC License/ ID;
4. Certified photocopy of Eligibility or Ratings obtained in the Licensure Examination for Teachers (LET)
5. Certified True copy of Transcript of Records (TOR) and Diploma
6. Photocopy of Certificate of Employment (COE), service record, performance rating, and school clearance for those with teaching experience from a private school
7. Certificate of Special Training, if any;
8. Certified copy of Voter's ID or any proof of residency;
9. NBI Clearance;
10. Checklist of Requirements and Omnibus Certification of Authenticity and Veracity of the documents submitted, signed by the applicants, and notarized by authorized officials.
