Eligibility:
1. Open to 2nd Year BS Biology, BS Mathematics, BS Statistics, and BS Computer Science students
2. Must have at least a Cumulative GWA of 2.00
3. A permanent resident of Visayas or Mindanao
4. Must not be a beneficiary of any other scholarships or educational grants
5. Must not have been the subject of any disciplinary action
6. Must be financially in need and whose gross annual family income from all sources is not more than ₱500,000.00

Requirements:

2. One (1) 2”x2” photo (to be attached to your application form)
3. Parent’s Income Tax Return (BIR Form 2316) from the previous year (2024) and other proof of income (Certificate of Employment and Compensation, Contract with Employer, Certificate of Indigency from the Barangay with income indicated); If applicant is working, his/her Income Tax Return and/or W2; If married, attach ITR and/or W2 of spouse; If income is derived from business, attach income statement; If exempted from filing, attach BIR Certificate of Exemption with a notarized affidavit source of income, indicate approximate income earned. If parent is unemployed, attach a notarized affidavit of unemployment, indicate approximate income earned and source of income/livelihood.

6. True Copy of Grades with computation of your cumulative GWA
7. Letter/s of recommendation from former teachers/professors (3)
