⬇️Here are a few steps to follow to have your Athlete Eligibility Form medically approved🏃‍♀️🏃‍♂️

Just make sure you:
⚽️have submitted medical requirements to the clinic upon admission to CTU and was rated as physically fit. (For 1st year students only)
🥎complied with medical advice, if applicable (taken iron supplements for anemic students, etc).
📋Completely fill out Athletes Eligibility Form
📋Completely fill out Athletes Logbook at the Clinic
🏀come to the Campus Clinic personally with your Eligibility Form
⚾ have no recent injury.
⚠️ failure to comply during these dates will be advised to secure their Medical Certificate from a gov't or private physician.
