PROGRAM
Basak Community Elementary School is accepting applications for its Special Program in Science (SPS) Program today May 5 until May 12, 2026.

We invite all interested and qualified learners to apply!

REQUIREMENTS:

* Letter of Intent (addressed to the School Principal, Mr. Jose B. Oliva)

* Learner's Profile (Form 3.2) With 2x2 colored picture (secure from the guard on duty)

* Declaration of Medical Condition Form (SPS Form 3.0)
(secure from the guard on duty)

*Medical Certificate from a doctor (to follow)

* Learner must be 6 years old on or before October 31, 2026

* Photocopy of ECCD Report and Kindergarten Progress
Report Card

* Must have an average rating of C (Consistent) in:
Language
Literacy and Communication
Mathematics
Understanding the Physical and Natural Environment (1st to 3rd Quarter)

*Certificate of Good Moral Character (for learners from
private schools or other divisions)

* Photocopy of Certificates of Academic and Non-Academic
Awards (arranged from latest)

* Photocopy of NSO/PSA Birth Certificate
