Announcement for all students. This survey is vital for the FHE Budget Hearing.

RE: “RAPID SURVEY ON PROFILE OF STUDENTS IN HIGHER EDUCATION.”

First of all, we would like to thank you for your support in the conduct of the “Rapid Survey on Reasons for Student Attrition in Higher Education.” Results were shared during the pre-plenary and plenary budget hearings of CHED at the House of Representatives. However, there were still questions asked during the plenary. These were student-level data that we usually do not collect, but may need to collect moving forward in order to determine the appropriate interventions or policies that need to be developed. Thus, in preparation for the budget hearing at the Senate scheduled in October 2023, the Office of Planning, Research, and Knowledge Management (OPRKM) is again conducting a new round of data collection which we call “Rapid Survey on Profile of Students in Higher Education.”
Please request your higher education institutions (HEIs) to participate in the survey by providing the link to the Google form to their undergraduate students. Moreover, please assure your HEIs that we adhere to Republic Act 10173 or the Data Privacy Act of 2012 and that no personal information of the students shall be shared with other parties. Data collected will be analyzed for the purpose stated above.

TES Payout Orientation 2021
All grantees MUST join the ZOOM webinar on Saturday, October 2, 2021. Guidelines and requirements during the payout will be discussed. Failure to attend the said orientation and incomplete submission of all the requirements is NOT AN EXCUSE during the payout.

List of Requirements
for Educational Assistance Payout

For students who have School IDs particularly the continuing or ongoing students:

1 Certificate of Enrollment (AY 2021-2022)
2 Certificate of Indigency (purpose: for whatever legal purpose it may serve)
3 School ID

For students who have NO IDs particularly 1st year and transferees:

1 Certificate of Enrollment (AY 2021-2022)
2 Certificate of No ID from the school
3 Certificate of No Valid ID from the barangay
4 Certificate of Indigency (purpose: for whatever legal purpose it may serve)

Note:
1 Please prepare Original and two photocopies of all required documents
2 Kindly affix 3 specimens of your signature under the photocopy of your School ID
3 Bring your own blue ballpen, face mask, face shield, alcohol, & hand sanitizer
4 Failure to comply with all the above-mentioned requirements means FORFEITURE
