ZNHS -Career Guidance Counseling and Consultation Services CGP aims to provide relevant information on the guidelines, procedures and activities related to career guidance program.

For this year, all career guidance orientations shall be held online in line with the (IATF) health standard protocols.

Instructions:
1. This enrollment survey shall be answered by the parent/guardian of the learner.
2. Please read the questions carefully and fill in all applicable spaces and write your answers legibly in CAPITAL letters. For items not applicable, write N/A.
3. For questions/clarifications, please ask for the assistance of the teacher/person-in-charge.
For Grade 7 & 11 Enrollees, You are required to UPLOAD a scan copy of the School Report Card and PSA Birth Certificate for you to complete the enrollment process.

List of requirements to be submitted within the first week of the Class for SY: 2021-2022:

1. School Report Card
2. Certificate of Good Moral
3. PSA/ NSO Birth Certificate
4. 2 pcs. 2x2 or 1x1 ID Picture (Formal attire and white background)
5. Certificate of Good Moral Character
6. Long Brown Envelope
