　　• Original Good Moral Certificate
　　• Original PSA Birth Certificate
　　• 2 pcs. 2x2 colored picture with blue background
　　• 2 pcs. long brown envelopes

Requirements:
　　• Original Certificate of Transfer Credentials
　　• Original Transcript of Records (TOR) for evaluation purposes (with the NSTP
serial number if NSTP 2 was completed in the previous school)
　　• Original Good Moral Certificate
　　• Original PSA Birth Certificate
　　• 2 pcs. 2x2 colored picture with blue background
　　• 2 pcs. long brown envelopes

Once enrolled, please proceed to the ICT office to get your ID. For school uniform measurements, proceed to the SAS office, except for Nursing, Pharmacy, and Criminology which will be facilitated by their respective colleges.
