To all Skillsians, please be informed that our releasing of academic records will be a "NO CLAIM STUB, NO RELEASE" POLICY. There are two ways to get your claim stub: Online & Walk-in.
1. For Online requests, you may fill out this Academic Records Request Form: https://forms.gle/Ein34VK5vdmx9JQv8 . You can receive the claim stub every Tuesday and Thursday which will be done by Ms. Ivy Canonayon, our Records-In-Charge. You may follow up through sending us a direct message.
2. For Walk-ins, our office will only be available Monday to Friday 8 AM to 3 PM. Please be guided on the Cut-off below.

Students' Clearance for First Sem & Enrollment Application for 2nd Sem in SY 2020-2021
Good day, SKILLS SHS students!
The end of the first semester is fast approaching. The first sem will end on December 18, 2020. Wish you luck in completing all your outputs and requirements in each SHS office. With this make sure to complete your requirements with the following offices/ personnel:
Faculty Office
- Subject Outputs
-Advisory requirements
Student Affairs Office (SAO)
- Training Attended
- Attendance of the First Sem Orientation
Guidance Office
- Training Attended
- Memorandum of Agreement Signing
Registrar's Office
- Enrollment Requirements

To all SKILLS Alumni,
You may now claim the Academic Documents (Hard copy). This procedure will be effective on September 21, 2020.
Kindly fill out the academic request form:

To the authorized person who will claim the documents, please make sure to bring the following:
1. Claim stab - a pdf/jpeg file sent to the requestor from the office of the Registrar indicating the control number, location, date, and time in claiming the documents. This will be sent after filling out the Academic Documents Request Form and if the document owner has no liabilities in any of the following offices:
- Faculty (T.I.C., subject and advisory teacher requirements)
-Library
- Accounting
- Clinic
- Guidance
- SAO
- Registrar
- Academic Director
2. SKILLS Student's school ID - must be surrendered.
3. Authorization letter
4. Valid ID of the claimant
We also need to ensure safety protocols should be followed upon entry at the building.
1. Wearing a face mask and face shield is a must.
2. Fill-out COVID 19 Screening Questionnaire.
3. Strict disinfection of hands at handwashing area.
4. Disinfection of walking shoes upon entry at the sanitizing bath area.
5. No sharing of ballpens.
6. Maintain social distancing at all times.
Claiming of academic documents is according to the schedule given to the requestor from the Registrar.
