2. Application Letter address to the Dean of the College of Education - downloadable
3. Photocopy of Transcript of Record and Diploma
4. Three Recommendation Letters (from former professors/administrator/employer) – downloadable
5. Receipt of payment for Application Fee (Pay 50 pesos as Application fee here: Land Bank of the Philippines, CLSU Income Account, Account No. 2961 0384 16)
6. Submit all scanned documents including the payment receipt (in one file) to this email: [email protected]
