💉For students who belongs to the Pediatric Group 12-17 years old - Birth Certificate
💉For Parents/Guardian - Valid ID
💉For Guardian/s Valid Id, Brgy. Clearances Certification/Any proof of Authorization from Parent.
💉No Parent/Guardian, NO VACCINATION
2. Bring Your Own Ballpen
3. Observe public health and safety protocols. Physical Distancing in and outside the rooms must be observed. No facemask no entry policy.
