UVFP History: The Foundations That Shaped Today’s Treatments

Dr. Erwin Payr (early 1900s)
Payr was one of the first to recognize that hoarseness in UVFP happens because the paralyzed vocal fold cannot move inward. He introduced the concept of medialization—physically repositioning the vocal fold to improve voice.
This was the first shift from “wait and see” to active treatment.

Dr. Nobuhiko Isshiki (1970s, Japan)
Isshiki refined Payr’s idea and developed laryngeal framework surgery, including Type I Thyroplasty, which is still a gold standard today.
His work allowed surgeons to improve voice without cutting the vocal fold itself, making outcomes safer and more predictable.
