- Must be 18 years old and above
- 4 pieces passport size photo
- 4 pieces 1x1 photo
- Photocopy of the ff:
*Birth Certificate (PSA/NSO/LRA)
*Marriage Certificate
(if a married woman)
*a copy of a Valid/Government
Issued ID(after photocopy,
please affix 3 signatures)
* College TOR:Form 137
or any school document
- Certificate of Indigency

Caps off to the Class of 2026!

Starting March 23 until April 30, QMLC office hours will be as follows:
Monday - online/by appointment
Tuesday to Friday - 8:00 AM to 3:00 PM.

The school office will be closed from March 31 to April 3 in observance of the Lenten Season. Regular operations will resume after.

Summer classes will begin on April 13, Monday to Friday, and will be conducted face to face.
