Heads up Unit Advisers, Coaches and Students!
Starting Sept. 14, 2026, the campus clinic has been accepting student-athletes for health assessment and eligibility screening for Intramurals 2026. This initiative was implemented to avoid overcrowding at the clinic, as agreed upon during the meeting held on September 3, 2026.
However, as of this date, no student- athletes have reported to the campus clinic for their health assessment.
All unit advisers and coaches are hereby advised to encourage all identified student athletes to visit us at the clinic as soon as possible to complete the required eligibility screening prior to their participation in the intramurals 2026.
Pls be guided by the following requirements:
- the Personal information portion of the Eligibility form must be completely filled out
- the eligibility form must be signed by the coach or teacher-in-charge. Unsigned forms will not be processed.
- students must accomplish the google form provided prior to their clinic visit.
