c. Portal will not be opened to students, you are only to accomplish the registration form.

1. The Enrollment shall be made through the google form only and ONLY the enrollment advisers will be enrolling students who accomplished the form.
2. Students with deficiency/ies will be emailed through their UB Email Account, thus, please monitor your emails regularly.
3. Registration Form will be opened on January 6, 2025 at 8:00 AM.

FOR SIHTM UB SCHOLAR STUDENTS
UB Performing Arts, Student Assistants & Athletes

Students must be ENDORSED by the UB Athletics Office (for athletes), OSA (for UB Graces, UBBUK, UB Orchestra, UB Band, & UB Voices), & HRMC Office (for Student Assistants). Strictly NO ENDORSEMENT, NO ENROLLMENT (make sure that your duty/training schedule is indicated in the letter and note that a yellow form is not an endorsement letter).

Visit the SIHTM Dean’s Office on January 8 - 11, 2025 from 8:00 AM to 4:00 PM.

**Enrollment for UB SCHOLAR STUDENTS will be through face-to-face ONLY.

Visit the SIHTM Dean's Office on January 8 - 11, 2025 from 8:00 AM to 4:00 PM. Please bring your evaluated curriculum with you (Curriculum Checklist evaluated and signed by the Dean’s Office personnel).

**STRICTLY NO EVALUATION, NO ENROLLMENT. Enrollment will be through face-to-face ONLY.

***Registration for students with unpaid school fees from the previous semester/school year for the new semester/school year and unsettled deficiencies from the Admissions and Record Center , SIHTM Dean‘s Office and SIHTM Stock Room is locked. These deficiencies should be settled before the registration of subjects.

Dear SIHTM Students,

Please STRICTLY enroll full block schedule (all subjects with the same section). For students who have back subjects (subjects to retake, or missed to enroll in the previous semester and are offered in different year level), pls add these subjects during the adding schedule ton August 7 to 11, 2023 ( except August 8 ). The enrollment advisers will be removing students who enrolled subjects taken from different sections, and will enroll these affected students under one block section only. Thus, please check your portal regularly for changes in the section of your enrolled subjects.
