Requirements:
-High School Card (Original and photocopy)
-Certification from HS Principal or Guidance Councilor
-School ID with picture
-1x1 and 2x2 picture
-black ballpen
-Entrance Examination Fee (P30.00)

Note: This is for non-fresh graduate high school students. Graduated in HS S.Y. 2014-2015 and below. Students who are not included to a K+12 program.
