For those who have already accomplished the online registration and signify their willingness to receive the 1st Booster Vaccination, please be informed that you are hereby confirmed for the ceremonial vaccination on October 10, 2022, 1:30 pm at the Bacolod City Activity Center Taculing Campus.
You are advice to see Nurse Zoe Ann Zamora at the Clinic Taculing Campus. In order for you to fill-up the Declaration Health Screening form.
