Usmad Elementary School Enrollment Schedule for SY 2024 - 2025 is July 3 - 26, 2024.

The said enrollment started last Monday, July 3 and thank you for those who enrolled but for those who are still planning to enroll, below are the requirements, enrollment processes, in-charge and some important announcements.💚

REQUIREMENTS:

🧡Kinder
*1 photocopy of PSA/NSO birth certificate or in the absence of Birth Certificate, Certification from the Brgy. of the basic information about the child must be complied
*must be 5 years old on or before September 30, 2024
*copy of ECCD if available

🧡Grade 1
* Photocopy of PSA/NSO birth certificate
* ECD Checklist
* Progress Report Card
* Photocopy of the Certificate of Completion

🧡Transferees
*Photocopy of PSA/ NSO
*SF 9 or Report Card / Certificate of Enrollment from the Previous School
*Good Moral Certificate
*Affidavit of Undertaking (if applicable)

🧡For K, Grade 1 and Transferees
Steps:
*Get a copy of the Enrollment Form from the Enrollment Officer of the Day.
*Fill-in the needed data.
*Submit all the required documents and enroll.

🧡For Grades 2-6
Steps:
*Get a copy of the Confirmation Slip from the Enrollment Officer of the Day.
*Fill-in the data needed data.
*Submit all required documents and enroll.

July 23- Kinder and Grade 1 parents

July 24- Grades 2 and 3 parents

July 25- Grade 4 and 5 parents

July 26- Grade 6 and 4Ps Members

July 27- All Homeroom Officers from K to Grade 5 of Last Year, Previous PTA-BOD Officers, SGC Officers, Alumni, Barangay LGU, Faculty and other stakeholders and volunteers
- Closing Program

MGA REQUIREMENTS SA PAG PA ENROLL:
👉*KINDERGARTEN:
Mga bata nga lima (5) ka tuig na o maglima (5) katuig pa kutob sa Oktubre 31,2022 (DepEd Order No.47,s.2016/Deped Order No. 20,s.2018) maoy pwedeng ma enroll sa kindergarten.
Palihog dad.a ninyo ang mga mosunod nga dokumento: Photocopy sa PSA BIRTH CERTIFICATE/LIVE BIRTH

👉*GRADE ONE:
Mga bata nga nakahuman na og Kindergarten o mga bata nga naa sa Kindergarten karon.
Palihog sa pagdala ani nga mga dokumento:
*Original Kindergarten Report Card
*Original ECD Checklist
*Photocopy sa PSA BIRTH CERTIFICATE/LIVE BIRTH

👉*TRANSFEREES:
Palihog sa pagdala sa mga mosunod nga dokumento:
*Report Card
*Certificate of Good Moral
*Photocopy sa PSA Birth Certificate

👉*OLD PUPILS:
*Report Card from previous grade level

📍*Among giawhag ang mga moadto sa atong tunghaan o eskwelahan nga mag pabilin sa pag obserbar o pagsunod sa atong health and safety protocols nga gipatuman sa atong eskwelahan.
