To proceed with your scholarship processing, please submit the following requirements to the OSDW Office on or before August 24, 2026.
REQUIRED DOCUMENTS:

Note: Please fill out ALL boxes using CAPITAL LETTERS ONLY.
2. Certificate of Enrollment (COE) - 1st Semester, AY 2026-2027
3. Barangay Certificate of Indigency
4. 1 pc. 2x2 ID Picture
REMINDERS:
1. If you are already a grantee of UNIFAST Scholarship Program or any NAtionally-funded scholar, disregard applying to this program,
2. Incomplete submission will cause delay in your scholarship.
