✅ Requirements
- Form 138 (SF 9 – Report Card)
- PSA Birth Certificate (photocopy)

📌 Mahalagang Paalala
- Magsuot ng tamang uniporme.
- Magdala ng sariling ballpen.
- Siguraduhin na mapirmahan ang inyong form sa bawat STEP bago pumunta sa susunod.

STEP 1: Filling Out of Form Kumuha ng enrolment form sa GUARD at sagutan nang kumpleto, lalo na ang CONTACT NUMBERS para sa sectioning updates.

Step 2: Evaluation of Documents Ipakita ang SF 9, Birth Certificate, at enrolment form. Siguraduhin na kumpleto ang mga detalye.

STEP 3: Encoding Ipasa ang form at dokumento sa mga gurong nakatalaga.

Step 4: Confirmation Slip Release.
