1. Duly filled-out and signed application form (printable after online application via doorstep.cmu.edu.ph)
Note: Signed by Student and Guidance Counselor or School Principal with 2 pcs 1x1 ID picture.
2. Vaccination card
3. Ballpen (blue/black ink)
4. Facemask and Alcohol
5. School ID for identification (if available)

2. Click register
3. Click CMUCAT
4. Click apply online
5. Click proceed to CMUCAT online application
