Step 1 - Sign in to Gmail account and accomplish FDSA Enrollment Form using the link below:

Step 2 - Follow the Instruction and Fill out the Student Information Sheet
Step 3 – For those who haven’t submit requirements please attach Scanned or digital copy (*jpeg, *jpg, or *png format) of the following credentials; or you can directly submit to the Registrar FB Page
a. Report Card/ Form 138 (applicable for Freshmen / Transferees)
b. Good Moral (if applicable)
c. 2x2 ID picture (white background)
Step 4 – Once information is accurately completed click on the “SUBMIT” subject for Admission Department approval and validation.

• To secure your enrollment slot, the applicant may pay the Reservation fee of Php 5,000.00 (College Students) through the following payment option
