1. Transcript of Records (for college graduate/undergraduate)/Diploma for High School (graduate/undergraduate)
2. PSA Birth Certificate or Marriage Certificate (if available)

5. Curriculum Vitae or Resume

3. Closed shoes. (Rubber or leather)
