4. Student’s Permanent Address: Recent address according to the Province, District, Municipality, Ward

For SEE 2078 (Recent G9)
1. Student’s Full Name (Proper Spelling)
2. Father’s Name (Proper Spelling)
3. Mother’s Name (Proper Spelling)
4. Student’s Permanent Address: Recent address according to the Province, District, Municipality, Ward

Bring those information on paper while coming to school to fill up the form.
