1. Online Form Submission Begins on: 24th Poush 2079 at 10 AM
2. Online Form Submission Ends on: 24th Magh, 2079 at 5 PM
3. Last Date of Bank Voucher Submission: 22nd Falgun, 2079 before 5 PM

B. Entrance Exam Dates:
1. MBBS : 24th Falgun, 2079 (11:00 Am to 2:00 Pm)
2. BDS : 25th Falgun, 2079 (11:00 Am to 2:00 Pm)
3. BSc Nursing, BASLP, B Perfusion Technology : 26th Falgun, 2079 (11:00 Am to 2:00 Pm)

C. Required Documents:
1. Citizenship Certificate ✅
2. National Identity Number (NIN) ✅ (This year NIN is Mandatory)
2. Recent pp size photo-1 ✅
3. Bank voucher / Online Payment Receipt ✅(Different Vouchers for Different Programs)
4. Transcript of +2 or equivalent ✅
5. Recommendation letter/s for reservation quota ✅
6. License from concerned council (if applicable) ✅

Note:
1. Date in voucher should be in B.S
2. If student has passed class 6-10 from government school then s/he should get recommendation letter from school and education wing of VDC / Municipality office.

Last date of Form Submission: 16th June, 2022

Note 1. Students willing to study MBBS / BDS in Bangladesh must submit this application form and form submission facility is available at Shah Educational consultancy.

Note 2: form is open only for 5 days starting from 12th June to 16th June, 2022. Since verification and payment processes takes time, you need to visit office.
