The use of ultrasound on the motor system, or musculoskeletal ultrasound (MSKUS), has increased in popularity within primary care over the last couple of years. This is not surprising, since the combination of clinical assessment, the (dynamic) use of MSKUS and functional movement learning is a domain specifically performed within the domains of physiotherapy, osteopathy, chiropractic and medical

doctors/physicians. MSKUS is used to visualize muscles, tendons, ligaments and sometimes nerves and blood vessels of the arms and legs to assess their quality, shape and function. Scientific research shows that clinical tests are often not specific enough and inadequate to come to a correct diagnosis. False positives occur often and it is important to limit these mistakes as much as possible.

𝐂𝐀𝐒𝐄 🔎 The subcutaneous calcaneal bursa, also referred to as the retro-Achilles bursa, retroachilleal bursa, or superficial retrocalcaneal bursa, is an adventitial bursa situated between the skin and the Achilles tendon. It is separate from the retrocalcaneal bursa, which lies between the Achilles tendon and the posterior aspect of the calcaneus.

The following case demonstrates severe degenerative tendinopathy with type 3 calcification, subcutaneous calcaneal bursitis located superficially to the tendon, and retrocalcaneal bursitis located deep to the tendon. Additionally, there appears to be the presence of a Haglund exostosis. 👀

👀 Gamekeeper's thumb refers to a sprain or tear of the ulnar collateral ligament in the thumb. A Stener lesion is a more severe variant where the torn ligament is displaced by nearby tissue, preventing proper healing. Treatment for gamekeeper's thumb typically involves splinting and physiotherapy. However, Stener lesions usually require surgery to reattach the ligament and restore function. Musculoskeletal (MSK) ultrasound is useful for distinguishing between gamekeeper's thumb and a Stener lesion.

👀 Webinar: Ultrasound Assessment of Midfoot Ligaments

Several ligaments around the Chopart joint, including the bifurcate, dorsal talonavicular, lateral calcaneocuboid, and plantar ligaments, are often missed in routine ankle ultrasounds. This is problematic as many patients have Chopart sprains, not typical lateral ligament sprains, leading to prolonged pain and delayed recovery. Accurate ultrasound requires a solid understanding of ankle and midfoot anatomy, including equally common but often-overlooked posterior and Chopart ligaments.

Did you know that calcification can migrate through the tendon into the bursa or bone? Migration of calcification in the rotator cuff refers to the movement of calcium deposits within the tendon. This can occur spontaneously or post-needling. It may lead to increased pain, inflammation, and altered shoulder mechanics, often seen in calcific tendinopathy. Ultrasound detects these changes, guiding management.
