⚠️ Clinical Reminder: A “Removable” Offloading Device Is Only Effective If It Is Actually Worn

A key evidence-based point for diabetic foot ulcer care:

When a non-removable knee-high offloading device is contraindicated or not tolerated, a removable knee-high or ankle-high offloading device may be used—but patients should be encouraged to wear it during all weight-bearing activities.

💡 This creates an important nursing and clinical care opportunity:

Instead of simply asking:
“Are you using your boot?”

Ask more specifically:
➡️ “When do you remove it?”
➡️ “Do you wear it for short walks at home?”
➡️ “Do you use it when going to the bathroom?”
➡️ “What makes it difficult to wear?”

Small periods of unprotected walking can undermine the goal of offloading.

👩‍⚕️👨‍⚕️ Routine practice point:
Offloading should be assessed as a behavior and adherence issue, not only as a prescription.

The best device is not simply the one prescribed—it is the one that is clinically appropriate, correctly applied, and used consistently during weight-bearing.

🦶 Diabetic Foot Ulcer Care: The Dressing Is Not the Whole Treatment

A clinical fact that can change routine practice:

➡️ For a person with diabetes and a neuropathic plantar forefoot or midfoot ulcer, a non-removable knee-high offloading device is recommended as the first-choice offloading treatment to promote healing.

Why does this matter?

Because every step can repeatedly apply mechanical stress to the ulcer. Even excellent wound cleansing, debridement, and dressings may not be enough if the ulcer continues to be loaded during walking.

Practical takeaway for clinicians and nurses:
When assessing a diabetic foot ulcer, ask not only:
“What dressing should we use?”

Also ask:
✅ “How is the ulcer being offloaded?”
✅ “Is the patient actually using the offloading device during weight-bearing?”
✅ “Is the device appropriate and safely applied?”

Evidence-based wound care is not only about what we put on the wound—it is also about reducing the mechanical stress that prevents healing.
