Vitamin K is an essential cofactor for the activation of coagulation factors II, VII, IX, X, protein C and protein S, via carboxylation of specific glutamate residues. Deficiency of this vitamin results in inadequate activity of these factors and consequently bleeding.
Hemorrhagic disease of the newborn occurs in 0.01 to 1.5% in the absence of vitamin K prophylaxis in the classical form, and in the late form in 4 to 10 per 100,000 births.

La vitamina K es un cofactor esencial para la activación de los factores de coagulación II, VII, IX, X, proteina C y proteina S, vía la carboxilación de los residuos glutamato específicos. La deficiencia de esta vitamina produce una inadecuada actividad de estos factores y como consecuencia hemorragias.
la enfermedad hemorrágica del recién nacido se presenta de 0.01 a 1.5% en ausencia de profilaxis de vitamina K en la forma clásica, y para la tardía de 4 a 10 por 100,000 nacimientos.

Male, 72 years old.
He came to the emergency room for gait instability with 5 days of evolution. He reported that the onset of the symptoms was abrupt, not accompanied by other neurological focality.

CT Brain
Left hemicerebellar hypodensity. Chronic ischemic lesions in basal ganglia, bilateral. Atheromatosis of the main intracranial arteries and ectasia of the left vertebral artery.

Evolution.
The patient worsens, develops dysarthria, hemiparesis and new onset right brachiocural hypoesthesia.

Diagnosis.

Masculino de 72 años.
Acude a urgencias por inestabilidad de la marcha con 5 días de evolución. Refiere que el comienzo de la clínica ha sido brusco, no acompañada de otra focalidad neurológica.

TC Cerebro
Hipodensidad hemicerebelosa izquierda. Lesiones isquémicas crónicas en ganglios basales, bilaterales. Ateromatosis de las principales arterias intracraneales y ectasia de la vertebral izquierda.

Evolución.
El paciente empeora, desarrolla disartria, hemiparesia e hipoestesia braquiocural derecha de nueva aparición.
